During the first session of play on day one of the second Test in the two-match series at the Newlands in Cape Town, Pakistan opener Saim Ayub suffered a twisted ankle while trying to stop a boundary in the outfield. After hitting the ground, Saim Ayub clutched at his leg, seemingly in a lot of pain before Aamer Jamal and Babar Azam attended to him, and then Pakistan medical staff arrived at the scene of the incident to treat him prior to helping him off the field.

The incident happened on the second ball of the sixth over when Ryan Rickelton steered a fuller length delivery from Mohammad Abbas towards the third man region. After Aamer Jamal managed to stop the ball from going to the boundary rope, he relayed it to Saim Ayub, who had ran behind his teammate in support, but suddenly, he lost his footing before tripping backwards, which led to his body weight falling entirely on his right ankle bending at an awkward angle.

After being attended by Aamer Jamal and Babar Azam for a few seconds, Saim Ayub, who was writhing in pain, got treatment from the Pakistan medical staff before being taken off the field. The right ankle of Saim Ayub was later bandaged before he was taken to the hospital in a stretcher, while his return to action is yet to be confirmed.
